THE INFORMATION ENVOY MAY COLLECT

The information Envoy may collect from you will depend on how you use the Site, as well as, your computer and web browser settings. There are essentially two types of information that Envoy may collect from your use of this Site. How Envoy collects and uses your information will depend, in part, on the type of information. One of these types of information does not identify an individual person (“Access Information”). The Access Information may include, without limitation, the type of device used, browser type, location, IP address, web site linked from, search terms used, pages viewed, links clicked, and time spent on particular pages or the Site as a whole.

Examples of volunteering Personally Identifiable Information on this Site include filling out a “Contact Us” form, or sending Envoy an email or audiogram through this Site. By submitting information on the “Contact Us” form, you agree that we may reply to you using regular email, which may not be entirely secure from interception. HOW WE USE INFORMATION

Access Information (defined above) cannot, by itself, be used to identify a particular individual. Therefore, we do not place any restrictions on the use of Access Information. Generally, this Access Information will be pooled together with Access Information from other visitors and then used to create reports on how our Site is accessed and used by our visitors. We may share or sell this Access Information with affiliates and third parties to better serve our visitors or further our business interests.

Personally Identifiable Information (defined above) can be used to identify a particular individual, so Envoy does place restrictions on how it is used. THIRD PARTIES

Your Personally Identifiable Information may be shared with a physician or other healthcare provider, if you request to be put in contact with a physician or other healthcare provider, if you are seeking more information about whether a product or service may be right for you, or if you or someone else’s safety or wellbeing is in question.

Your Personally Identifiable Information may also be shared with companies that provide services or functions for Envoy in Envoy’s ordinary course of business.
