To qualify for the Esteem FI-AMEI you must be age 18 or older. Rule out, end test, no form.
To qualify for the Esteem you must have bilateral hearing loss. Rule out, end test.
The Esteem is designed for people with sensorineural hearing loss. If you have another type of hearing loss such as conductive hearing loss or mixed hearing loss you would not qualify.
